  • You can contact the Appointments & Information department by phone to make or change appointments.
  • You must inform the Appointments & Information department in writing of any change of address, or of a change in your health insurance arrangements. This is to avoid errors - as far as possible - when noting down data.

When registering, each patient will receive an ACTA patient card, bearing contact information. Details of future appointments will be entered on the inside of your card. A sticker bearing your personal patient number is attached to the back of this card. Keep details of this number handy when contacting ACTA to help us provide you with the best possible assistance.
